History
Founded in 1984 originally as a nurse staffing company for the San Francisco Bay Area under the name Nurse Providers, the company has grown to become one of the western United States’ leading home care agencies providing a full line of home health services, including in-home skilled nursing and rehabilitation, as well as private duty shift nursing and non-medical care. In 2006, Nurse Providers, now focused entirely on home-based non-medical care, bought Ablecare, another private duty agency, and then merged with Professional HealthCare At Home, a licensed, Medicare-certified home health agency founded in 1993. Under the name Professional HealthCare At Home, the company now operates 10 branches across northern California. In early 2008, the company purchased Haven Home Health (Salt Lake City, UT) and continues to serve the greater Salt Lake area through the Haven Home Health brand. In mid 2008, the company purchased Victorian Home Care (Monterey and Santa Cruz, CA), a private duty agency with offices in Monterey and Aptos, California, and continues to serve those communities under the Victorian name.
Professional HealthCare At Home is focused on expanding its services across the western United States. Despite our growth, we are committed to maintaining the highest levels of quality care and professional ethics. We are a Medicare and MediCal/Medicaid certified home health agency licensed by the States of California and Utah. We were also awarded "deemed" status by the Community Health Accreditation Program (CHAP), which is the highest rating for a home care agency.

Ourservice area includes San Francisco as well as Oakland, Berkeley andAlameda County, Walnut Creek, Roosmoor, Concord and Contra CostaCounty, Daly City, Redwood City and San Mateo County, Sunnyvale, SanJose and Santa Clara County, and Vallejo, Fairfield and Solano County.Services include nursing care, physical and occupational therapy,post-hospital rehabilitation, extended care, wound care, home healthaides, medical social workers, medication management, senior care andhospice care. Home caregivers and peronal care assistants provide helpwith activities of daily living, eldercare, transportation andcompanionship.
